Social Prescribing

Social prescribers focus on non-medical and non-urgent support for those that require emotioanl and social help, such as:

  • Housing Support
  • Benefits Entitlements (filling forms such as PIP)
  • Bereavment Support
  • Counselling
  • Befriending Service
  • Finding Suitable Social Groups
  • Referral to Social Services (E.g. Care assessment)
  • Supporting family carers
  • Dementia Support
  • Home Adaptations for those in need
  • Alcohol/Drug related support
  • Weight Management
