New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. You must live within the area covered by our practice, see below, please call Reception to enquire if you live within our boundary.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

REPEAT MEDICATIONS

All new patients on repeat medications must have an appointment with a GP prior to being issued with any repeat medications, this appointment can be made at time of registering, please ensure that you have sufficient medication from your previous surgery prior to moving to us as we unable to issue repeat prescriptions for new patients until after the GP appointment.

Our process for registering new patients:

We would firstly ask you to call the surgery or pop in and confirm that you are living within our catchment area.

We can then provide you with a New Patient Information Pack, Practice Leaflet and GMS1 Form or this can all be printed from the links below. 

We ask that when you come to register that you have all the correct documents and ensure that the Adult Health Questionnaire and GMS1 Form have been fully completed. We are unable to register you until you provide all the necessary documents.

Children do not need to be present for the registration as all documents will be completed by a parent/guardian on their behalf, however, you will need to bring their completed Under 16 Health Questionnaire form and GMS1 Form with you and their Red Book.

Adults must come in and register themselves.

We do not register any children at the practice without a parent/guardian being registered here.

Temporary Registration

Immediate Necessary patient

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency immediate necessary treatment from the local GP practice. After 14 days you will need to register as a temporary or permanent patient.

Temporary patient

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

If you register as a Temporary patient and wish to see a doctor you will be offered the next available routine appointment.
